Due to State and Federal Guidelines and requirements, the college must keep the legal name in its student and Employee Information Database currently called Colleague.

 
  1. https://www.ccac.edu/about/forms.php#registration-and-advising-forms
  2. Select "Legal Name Change" 
  3. Fill in your name 
  4. Fill in your email
  5. Click the "Begin Signing" button
  6. Check the Checkbox agreeing to using an electronic signature
  7. Click the "Continue" button
  8. Fill in your Colleague ID Number (Locating your 7 digit Colleague number)
  9. Fill in your First Name as it is in the system
  10. Fill in your Middle Name as it is in the system
  11. Fill in your Last Name as it is in the system
  12. Fill in you current Address
  13. Fill in your current City
  14. Fill in your current State
  15. Fill in your current Zip Code 
  16. Fill in your current phone number
  17. Click all status that apply
  18. Fill in your corrected name
  19. Select the Reason for the name change
    1. Misspelled/Typo Correction - for example Smyth to Smith you will be required to attach a photo id that has the correct spelling on it.
    2. Legal Name change - your will be required to attach a photo ID and the legal documentation that validates the name change for example Marriage License or Court Decree
  20. To attach your photo id, click the yellow button with a red paper clip
  21. On the pop up, click "Upload a File" button
  22. Select the correct file and click the "Open" Button on the windows pop-up
  23. Click "Done" button on the pop-up (your should now see your filename on the pop-up)
  24. You should now see your attached document added to the DocuSign form 
  25. To attach your legal Documentation, click the second yellow button with a red paper clip
  26. On the pop-up, click the "Upload a File" button
  27. Select the correct file and click the "Open" Button on the windows pop-up
  28. Click the "Done" button on the pop-up (your should now see your filename on the pop-up)
  29. if you selected "Current Student" above, you will need to answer the question if you would like your ccac username and email updated to reflect this change.
  30. Click the yellow "Signature button"
  31. Set up your digital Signature on the popup, and click the "Adopt and Sign" Button when you are done
  32. Now Click the "Finish" button and the system will route your form to where it needs to go
  33. On the pop-up, you can download or print a copy for your records which is optional, hit the "Close" button to be done
  34. You will receive an email from DocuSign when the form is completed. Please note it may take one day for the change to go from Colleague to other college systems after you receive the completed email and that username changes may be delayed due to update constraints.
