BEFORE YOU BEGIN

You will need:

  1. From a web browser, go to https://portal.castlebranch.com/yc88
  2. Click PLACE ORDER button
  3. Expand the Please Select
  4. Expand the Student
  5. Click YC89realid: CB Real Vaccination ID for CCAC Student 
  6. Read the order instructions
  7. Click the checkbox to approve you have read the instructions
  8. Click the Click to Continue button
  9. Read the overview
  10. Click the checkbox to approve you agree to the terms
  11. Click the Continue button
  12. Enter your 7-digit CCAC number as the PIN
  13. Click the Next button
  14. Enter your information 
  15. Click the Next button
  16. Create a Password and retype in Confirm field
  17. Click the Create Account button
  18. Check the name is selected from the drop-down menu
  19. Click the Next button
  20. Check over the information and ensure it is correct
  21. Click the SUBMIT button
  22. A Thank You screen will appear
  23. Click the Next button
  24. Complete the profile
  25. Click Next button
    You may need to scroll down
  26. Complete the profile page
  27. Click Next button
    You may need to scroll down
  28. Complete the profile page
  29. Click Next button
    You may need to scroll down
  30. Create security questions
  31. Click Complete button
  32. Answer No
  33. Click the Continue button
  34. Click the Finish button
  35. Click the Go button

Submitting Documents to Castle Branch

BEFORE YOU BEGIN

You will need:

  • A photo of yourself and personal identifiers - Driver's License or Passport
    • A school issued ID card will be accepted in lieu of a government issued photo ID
  • A current photo of you with a solid background (this will be your picture on your ID)
  • Proof of all required doses of an available COVID-19 vaccine

If you have used Castle Branch before, you may have these documents in the Document Center and can reuse them.

  1. Click the + to expand the YC89 Real Vaccination ID
  2. Click the + to expand the Consent to Share
    1. Scroll to read
    2. Click the checkbox to complete
    3. Click the Submit button
    4. It will be now marked complete
  3. Click the + to expand the Attestation
    1. Read the text
    2. Click the checkbox to complete
    3. Click the Submit button
    4. It will be now marked complete
  4. Click the + to expand the Height and Eye Color
    1. Type in your height and eye color in the field
    2. Click the Submit button
    3. It will be now marked Pending Review
  5. Click the + to expand the Upload Copy of Government Issued
    1. Click the Browse from the Your computer or flash drive section
      If you already have this document uploaded, use the Browse button across from My Documents
    2. Locate the photo on your device
    3. Click the Submit button
    4. The submission process will show
    5. It will be In Process, then Pending Review
  6. Click the + to expand the Upload Copy of Recent Photo for Real Vaccination ID Card
    1. Scroll down to read
    2. Click the Browse from the Your computer or flash drive section
      If you already have this document uploaded, use the Browse button across from My Documents
    3. Locate the photo on your device
    4. Click the Submit button
    5. The submission process will show
    6. It will be In Process, then Pending Review
  7. Click the + to expand the Proof of COVID-19 Complete
    1. Scroll down to read
    2. Click the Browse from the Your computer or flash drive section
      If you already have this document uploaded, use the Browse button across from My Documents
    3. Locate the photo on your device
    4. Click the Submit button
    5. The submission process will show
    6. It will be In Process, then Pending Review
  8. Once completed, the first 3 should be completed and the last 3 should be Pending Review. Which Castle Branch will review.

    3. If you already have this document uploaded, use the Browse button across from My Documents
    4. Locate the photo on your device
    5. Expand the OPTIONAL: COVID-10 Booster section
    6. Click the Submit button
    7. The submission process will show
    8. It will be In Process, then Pending Review


Logging into Castle Branch

  1. From a web browser, go to https://login.castlebranch.com/login
  2. Use the email and password you used to create the account
  3. Click the Sign In button
  4. Click the Go button
