Add/Drop/Withdrawal Form – to be used when a student needs to make a change to their schedule but are unable to access self-service.

Audit Agreement Form - to be used when a student wants to audit a credit course

Diploma Order Form– to request a copy of your CCAC Diploma

Legal Name Change Form – to be used when a student needs change their legal name in the College's information system..

Registration Form – to be used when a student who has already applied and cannot register online, needs to register for classes

Residency Appeal Form – to be used when a student needs to have their residency changed for a future semester

Senior Citizen Registration Form – to be used when a student needs to register with a senior citizen waiver.  CCAC permits senior citizens, defined as a person 65 years of age and older, who are residents of Allegheny County to enroll in up to two regularly scheduled credit courses each semester without payment of tuition charges
